Enrollment
Are there other ways to enroll?
Yes. You can call 844-515-5873 toll free (Monday through Friday, 8 a.m. to 9 p.m., EST) and a friendly representative will assist you by mailing you the form you need to enroll.
Can I get insurance for my spouse and family, too?
Simply select the family plan, it will automatically cover you, your spouse or domestic partner and all your dependent children. Your daily In-Hospital, emergency room, and Intensive Care Unit benefits will be the full amount you have selected on your enrollment form. Your spouse or domestic partner will receive 50% of this amount and each of your dependent children will receive 20%. If there are no eligible children, your spouse or domestic partner will receive 60% of your amount; if there is no spouse or domestic partner, each dependent child will receive 40%.
Do I have to take a medical exam or answer a lot of health questions?
No medical exams, blood tests, or health questions are required to get and keep this coverage.

Federal Insurance Company, a Chubb company, is the underwriter.
When does coverage begin?
Your effective date will be shown on your certificate/policy of insurance that you will receive within 15 days of our receipt of your activation form. Please note that your effective date may be delayed depending on the billing cycle of your financial institution. Please be sure to review your certificate/policy of insurance upon receipt to determine your effective date.
Are there any additional benefits?
You automatically receive the following benefit (Except in MN):
Inflation Protection — for each year from the plan effective date, the In-Hospital, Intensive Care, and Emergency Room Benefits will increase by 10% for as long as your coverage is in force until it reaches 150% of the original amount. Please refer to the Summary of Benefits below, or once insured, your Insurance Coverage Document for specific details.
What exclusions apply?
Exclusions may vary based on your state of residence. Please review the Summary of Benefits for the exclusions in your state.
Who is covered under the Family Plan?
The Family Plan would provide coverage for you, your spouse or domestic partner and all your dependent children. Please refer to the Summary of Benefits for more detailed information.
Is there an age cutoff?
There is no termination age; however, confinement benefits reduce by 50% at age 65 and reduce to 25% at age 75 (except in MN). Your Outpatient Emergency Room benefits do not reduce due to age.
What if I have other insurance?
This insurance will pay all cash benefits due to a covered accident regardless of any other insurance that you may have.
Are on-the-job accidents covered?
Yes. Your coverage is effective 24 hours a day, anywhere in the world.

Billing
How will I be billed for my coverage?
The coverage you select will be conveniently billed to the account provided at the time of enrollment.
What day does the premium come out of my account?
On the 5th day of the month unless it falls on a weekend or holiday.
What if I am not happy with this plan?
You have a no-risk, 60-Day, 100% Money Back Guarantee. If you are not happy with the insurance for any reason, and you have not submitted a claim that has been paid, or will be paid, simply contact us within the first 60 days of enrollment for a complete refund of your premium and the coverage will be canceled back to its effective date.
Claims
How do I file a claim?
You can request our Insurance Claim Forms by calling our Claims Department at 800-252-2148. A representative will be glad to assist you in filing a claim.
Why do you need my hospital records to process a claim?
These records are required by the insurance company to verify the primary cause of hospital confinement was due to a covered accident.
What if I can't get an accident report?
Submit all information that you have relevant to the accident, including a police report if there is one, and the insurer will contact you if more information is needed.
