This insurance provides Accidental Death and Dismemberment benefits in the event of loss of life, limb, sight, speech, or hearing as the result of a covered accident.
What is not covered:
This Insurance does not cover any claim arising out of bodily injury caused or contributed to by any of the following unless coverage is specifically provided for by name in the Schedule of Benefits:
1) Intentionally self-inflicted Injury, suicide or any attempt thereat while sane or insane; for CA and MO residents: intentionally self-inflicted Injury, suicide or any attempt thereat; for SD residents: intentionally self-inflicted Injury, or any attempt thereat while sane or insane; suicide while sane, within two years of the effective date of your certificate; 2) commission or attempt to commit a felony or an assault*; for CA and NE residents: commission or attempt to commit a felony; for NJ residents: loss to which a contributing cause was the commission of or an attempt to commit a felony or to which a contributing cause was the engagement in an illegal occupation; for OR residents: commission of a felony for which the Covered Person is convicted; for UT residents: voluntary commission of a felony or an assault; 3) commission of or active participation in a riot or insurrection*; for NE residents: being engaged in an illegal occupation; for UT residents: voluntary commission of or active participation in a riot or insurrection; 4) Bungee-cord jumping, parachuting, skydiving, parasailing, hang-gliding, scuba diving, rock or cliff climbing, kite surfing, racing, professional sports; 5) declared or undeclared war or act of war; for OK residents: declared or undeclared war or act of war while serving in any Armed Service or its auxiliary; for VA residents: declared or undeclared war or act of war. For purposes of this exclusion, war means any act of conduct resulting from war, declared or undeclared or warlike action by any individual, government, military, sovereign group or other organization. It does not include terrorism or terrorist groups; 6) flight in, boarding or alighting from an Aircraft, except as: a. a fare-paying passenger on a regularly scheduled commercial or charter airline; b. a passenger in a non-scheduled, private Aircraft used for pleasure purposes with no commercial intent during the flight; c. a passenger in a military Aircraft flown by the Air Mobility Command or its foreign equivalent; 7) travel in or on any on-road and off-road motorized vehicle that does not require licensing as a motor vehicle*; 8) participation in any motorized race or contest of speed*; for GA and NE residents: participation in any organized motorized race or contest of speed; for ID residents: participation as a professional in any motorized race or contest of speed; 9) Sickness, disease, bodily or mental infirmity, bacterial or viral infection or medical or surgical treatment thereof, including exposure, whether or not accidental, to viral, bacterial or chemical agents except for any bacterial infection resulting from an accidental external cut or wound or accidental ingestion of contaminated food; for ID residents: the exception for bacterial infection does not apply to the Accident Medical Benefit; for KS residents: Sickness, disease, bodily or mental infirmity, bacterial or viral infection or medical or surgical treatment thereof, including exposure, whether or not accidental, to viral or bacterial agents except for any bacterial infection resulting from an accidental external cut or wound or accidental ingestion of contaminated food; for SD residents: Sickness, disease, bodily infirmity, bacterial or viral infection or medical or surgical treatment thereof, including exposure, whether or not accidental, to viral, bacterial or chemical agents except for any bacterial infection resulting from an accidental external cut or wound or accidental ingestion of contaminated food; 10) release, whether or not accidental, or by any person unlawfully or intentionally, of nuclear energy or radiation, including Sickness or disease resulting from such release* (not applicable to IL or KS residents); for MT residents: release, whether or not accidental, or by any person unlawfully or intentionally, of nuclear energy or radiation, including Sickness or disease resulting from such release, unless such release is caused by or is an act of terrorism; 11) the Covered Person being legally intoxicated as determined according to the laws of the jurisdiction in which the Covered Accident occurred* (not applicable to NV residents); for MI residents: your being legally intoxicated as determined according to the laws of the jurisdiction in which the Covered Accident occurred during commission of or attempted commission of a felony or assault or while engaged in an illegal occupation; for NJ residents: loss sustained or contracted as a consequence of your intoxication or being under the influence of any narcotic, unless administered or consumed on the advice of your Physician; for SD residents: your use of narcotics or drugs, or being legally intoxicated as determined according to the laws of the jurisdiction in which the Covered Accident occurred, in connection with the commission of a felony; 12) voluntary ingestion of any narcotic, drug, poison, gas or fumes, unless prescribed or taken under the direction of a Physician and taken in accordance with the prescribed dosage* (not applicable to SD residents); for NV residents: voluntary ingestion of any poison, gas or fumes; for OR residents: voluntary ingestion of any illegal narcotic or drug; 13) a Covered Accident that occurs while on active duty service in the military, naval or air force of any country or international organization. Upon Our receipt of proof of service, for GA residents: Upon Our receipt of written proof of service within 31 days of the date such service begins), We will refund any premium paid for this time. (for MO residents, We will refund a pro-rata unearned premium paid for this time; for PA residents: We will pay a pro-rata refund of any premium paid for this time). Reserve or National Guard active duty training is not excluded unless it extends beyond 31 days*; 14) Operating any type of vehicle or conveyance while under the influence of alcohol or any drug, narcotic or other intoxicant including any prescribed drug for which the Covered Person has been provided a written warning against operating a vehicle or conveyance while taking it (for MI residents: during the commission of or attempt to commit a felony or assault or engagement in an illegal occupation; for SD residents: in conjunction with the commission of, or attempt to commit, a felony.) Under the influence of alcohol, for purposes of this exclusion, means intoxicated, as defined by the motor vehicle laws of the state in which the Covered Loss occurred* (Not applicable to NJ residents); for NV residents: Operating any type of vehicle or conveyance while under the influence of any prescribed drug for which you have been provided a written warning against operating a vehicle or conveyance while taking it. 15) For OR residents: Voluntary inhalation of any poison, gas or fumes unless such exposure occurs during the course of employment; 16) Services or treatment rendered by you or any person who is: a) employed or retained by the Policyholder; b) living in your household or c) your parent, sibling, spouse or child.
*For ID residents: these exclusions do not apply to the Accident Medical Benefit.
